fallen out on me once, but I think I had put it in with the bulky end towards the bottom, I have started making sure I slide it in with the bulky end towards the top and it doesn't seem like it will slide out now. I think over time the ratchet driver might stretch out the material and eventually get loose over time, but if I put in the same way every time I think it will hold up. The only suggestion I would make would be to have some type of bottom "pocket" that the extension or ratchet driver could bottom out in and prevent them from sliding out.
